How to Refer
Referring to CPC-RPA
This clinic serves community patients from Redfern CMHC, Camperdown CMHC, and Marrickville CMHC. In addition, direct GP referrals are seen at this clinic site. Outside of SLHD referrals (in non-Covid times) are also seen here.
- If you are an SLHD care coordinator, the eReferral system is preferred. However, paper referrals are acceptable in 2023.
- If you are a GP who has been linked to our Direct referral system from your practice computer, use the Healthlink smart form.
- If you are a GP working in shared care with SLHD, please send the completed GP referral form to the SLHD care coordinator for inclusion in her/his referral to ccCHiP.
- If you are an outside GP, please download the pdf referral form and
email to
referrals AT ccchip.clinic
(replace AT with @ sign).
Referring to Concord Medical Centre
This clinic serves community patients from Croyden CMHC, Canterbury CMHC, and occasionally Marrickville CMHC. In addition, referrals from outside of SLHD close to the western boundaries of the SLHD catchment may be reviewed here (in non-Covid times).
- If you are an SLHD care coordinator, the eReferral system is preferred.
- If you are a GP who has been linked to our Direct referral system from your practice computer, use the Healthlink smart form.
- If you are a GP working in shared care with SLHD, please send the completed GP referral form to the SLHD care coordinator for inclusion in her/his referral to ccCHiP.

Referring to community health centres
This clinic serves community patients from each of the four community centres it rotates between: Redfern, Marrickville, Croydon, and Canterbury. These clinics take only referrals of patients who have a care coordinator with a community team.
- These clinics are specifically designed to see people who for reasons relating to their mental illness such as fear, trauma or disorganisation, are very unlikely to be able to attend either CPC-RPA or Concord Medical Centre.
- As there are very few appointments in these clinics across a year, we cannot offer these appointments to people who would simply prefer to attend their community centre.
- If you are an SLHD care coordinator, the eReferral system is preferred.
Referring to Sleep clinicians
Invitations to the ccCHiP Sleep Clinic are organised by our sleep coordinator/s. These clinics are for patients who have already attended a ccCHiP clinic and have been identified as being at risk for sleep-disordered breathing.
We may ask you to complete the Sleep Clinic Referral Form below to facilitate your patient’s review by the Sleep Clinicians.
